Claim Section

The Claim section allows you to create, edit and send the claims and charges for the patients.

The Notes tab is commonly used to track your day-to-day interactions with a specific claim. CollaborateMD supports entering two types of notes related to a claim, this includes Claim Notes and Follow Up Notes. 

Claim Notes are commonly used to enter general information regarding a claim. For example, if a claim was rejected by the Clearinghouse due to an invalid ICD Code, a best practice is to enter a claim note that summarizes the incident followed by the action that was taken. 

Sample Note:

The claim was rejected because the ICD-10 Code (I70) wasn't coded to the highest level of specificity. This code has since been updated to I70.233 and I inactivated this code within the Code section. Emailed the team so they were aware of this change.

Follow Up Notes are commonly used to document all interactions with when verifying the status of a claim. Best practice is to fully document all conversations with payers or patients. When documenting your notes, clearly transcribe your findings.Fully document each interaction that you have with the claim in addition to any conversations that you may have with the patients or payers. Summarize the current status of the claim followed by the next actions.

Spoke with Suzane, she confirmed that the claim was received on 7/1/2016. According to their system, this patient does not have benefits for the DOS submitted, however, the patient has paperwork that states otherwise. Contacted the patient and requested for her to fax me her verification of benefits letter. Once received I will contact the payer. I will create a follow up task for 2 business days from today. Once received I will call the payer

Notes 

Find Note

Follow the instructions below to locate a Claim Note or a Claim Follow Up Note:

  1. Go to the Claim section.
  2. You can search for claims in one of four ways:
    1. Use the Search field provided to search by patient name, phone#, and more. Click on the (?) for all search capabilities.
    2. Click on Find for <Patient Name> button to find claims for the active patient.
    3. In the Recently Opened pane, select the claim you wish to open. 
    4. Click on the Advanced Search button. 
  3. Click on the Notes tab. 
  4. Click on the sub-tab that best suits the note you're searching for. 
  5. When you're satisfied with your changes click on the Save button to save the changes you made to the claim.

Add Note

Follow the instructions below to add a note to your claim:
  1. Go to the Claim section. 
  2. Add or search for an existing claim to open.
  3. Click on the Notes tab.
  4. Click on the Claim Notes sub-tab and click on the Add Note button.
  5. Click on the sub-tab that best suits the type of note you wish to enter.
  6. When you're satisfied with your changes click on the Save button to save the changes you made to the claim.

Delete Note

Follow the instructions below to delete a note from your claim:
  1. Go to the Claim section. 
  2. Add or search for an existing claim to open.
  3. Click on the Notes tab.
  4. Click on the sub-tab that best suits the type of note you wish to delete.
  5. Check the box in the Delete column beside the note you wish to delete. Note: Once the note is deleted it cannot be recovered. 
  6. When you're satisfied with your changes click on the Save button. 
Note: Only Admins, Authorized Reps, and the person who created the note can delete it.
