FL Shots

Florida SHOTS (State Health Online Tracking System) is a free, statewide, centralized online immunization registry that helps health-care providers and schools keep track of immunization records. The easy-to-use data upload functionality allows health-care users to transmit all of their immunization histories from CollaborateMD into the FL SHOTS data base in under 4 minutes. Complete the steps below to begin configuring your FL SHOTS account:

Step 1 - Enrollment

Visit to enroll for free. Please contact the Florida SHOTS help desk if you have any questions. Once enrolled  FL SHOTS will provide you with an Organizational Login ID. Note: This value is received in the HL7 message, specifically the MSH 4.1 segment. Please note it can take up to 3 business days to be enrolled as the enrollments desk must verify the medical license.

Step 2 - Configuration 

Once you have your Organization Login ID, you must add this ID into the Practice section of the application. Follow the instructions below to add the Organizational Login ID

  1. Go to the Practice section. 
  2. Click on the Show All button and select the practice (if applicable). 
  3. Once the Practice is displayed, locate the FL Shots ID field and enter the Organizational Login ID (See the shot below).
  4. When you're satisfied, click on the Save button to save your changes. 

Step 3 - Request Integration

Now that you've entered your Organizational Login ID please fill out the Interface Request Form and select FL Shots from the list for the integration to be enabled. Please note it can take up to 3 business days for the integration to be enabled. Once enabled you will be notified via email by a CollaborateMD representative
